If you or someone you know is critically ill, recovering from an accident or surgery and is in need of "spirit lifting", this is the place!

Please fill out all information below and be sure to list parent information in the "Special Info" box if the recipient is under 18. If you have any questions, email Susan Farr angels2theheart@gmail.com 

**If you are filling this out for someone other than you, please give your complete info in the "contact person" fields. You will be notified if the recipient is accepted to our program. After you are notified that they are accepted, you MUST let them know.

**You will be required to comply with a monthly update in order to keep us updated on the recipient's condition and needs. If you do not comply with the update requests, the recipient will be removed. 

**We are not able to accept all applicants. Applications are processed according to severity of need. Please make sure you fill our application completely and properly. Sometimes the recipients will be placed on a waiting list until we have an opening for new recipients, so please be patient and know that not all applications can be accepted.

**Please remember that our recipients are NEVER expected to reply to their angel mail. We don't want this burden on them, and only want to lift their spirits!  

**Efforts to fraudulently use Angels2TheHeart for personal gain will be reported and fully prosecuted.
